for the same patient as picture shown Anying as water, food given, patient vomit...... means not taking and facing swallowing difficulty with mouth dryness as liver bilirubin always causing vomiting so
for hydration 500ml NS 0.9% given for 1hr at 50ml/hr thereafter given IV INJECTION metoclopramide 10mg ( 2ml ampules ) for vomit
then added IV 3 chamber bag diet as given below after 1hr Ns infusion....... then both NS 0.9% and Iv feed bag given by the rate 50ml/hr till finish
All the above step help patient from vomiting and tiredness feeling from nausea....... IV food given when he was nill per oral (Vomit if given)
at home for vomiting medicine used for following
Vomiting :- tab Vomikind md 4 or Ondem md 4.......for severe vomiting and nausea injection Emeset 4ml ampules ( each ml contains 2mg ondensetron )IM given... not cannula available
tramadol hydrochloride 100mg injection in 2ml ampules... im given checking air bubbles
drains in lock mode

Two large tumor on liver
due to which skin dryness as well as skin breakdown lesions on only both legs
both Ointment BID used
mupirocin ointment was being used which was also helpful but above mensioned is being used now a days
paste over gum bleeding (for teeth use only as written in dairy)
for diarrhea 3-4 stool passes Redotil capsules ( Racecadrotil 100mg maximum 3 times before food can be given )
Or IMMODIUM capsules ( loperamide 2mg OD if necessary BD can be given)
Enterogermina respules BD used
if constipation means difficulty in stool passes if go washroom 2-3 times but not passes...
tab cremalax 10mg used ( 6-8 hours working time)
if feeling pain due to constipation.... give suppositories (give 1 wax.........maximum 3 wax can be given )after 30 minute to 1 hour 30 minute stool not passes...... give enema by using lox 2% jelly to prevent bleeding insertion
hepagard sachet used, increased bilirubin to lower OD used

Buscopan 20mg injection im Bd given :- hyosine butylbromide( abdominal bloating and severe pain...as spasmodic )

More aspirations cough which cause SPO2 drop
Respiratory rate 15 - 35
Sodium level low than normal
Levetiracetam iv 100mg for low sodium or Levetiracetam iv changed into tab oxetol 300mg bd
Dex med 40mg injection used for sleeping as well as asthma
Doxycycline 100mg iv for cough and fever
Addphos 3.2g( Sodium acid phosphate granules to increase phosphorus for constipation bd used)
When oxygen is droping below 85....and no stable oxygen then hand palm gets cooled....
BD USED THIS RESPULE
2ml NS + 1ML HEPARIN FLUSH FOR PICC LINE flushing if doctor prescribe....

Due to cough and fever spo2 was like 88, 89,90....after appling Doxy forte 100mg, cough and fever relief as 98.6 F and Spo2 found as 94, 95%.
When patient Spo2 was below 88 %, Oxygen at 1 level was given..... But Doxy forte for fever started change found as above....
When crackle sounds was finding , Ceftum (cefuroxime 500mg ) was given for 3-5 days..... No cracle found after 5 days no Ceftum.... At first CRP:- CREACTIVE PROTEIN which represent blood infection was 162 after 7 days it became 44 and now CRP changed into 126 which represents infection having fever as 100F....due to infection swelling in leg and all the body are with obese....

patient has no motion after given last night cremaffin liquid , Softgel powder ayuvedic 2 teasponsful at same sime is having pain and went at hospital at 2pm Then Pruvict 1 given + Laxopeg (water soluble sachet) but motion not passed thenSuppository enema ( batti enema) given, after 4 hours motion not passed then Liquid enema given
